If your Mums, Dads Grandpa's and Mars, sisters, brothers, boy or girl friends are going to Guernsey to support you then there is a spectators accreditation/registration form that if completed will allow your support team to get a spectator pack, have free bus transport and entry to museums, there is a cost of GBP 20. If you wish to take advantage of this facility then confirmation must be in by the 26th April with the fee. I will need to complete the form electronically on your behalf therefore please can you send me the details as noted below ASAP.

• Gender.
• D.O.B.
• Home address.
• Place of birth.
• Home telephone number.
• Phone number (During games; suggest mobile or hotel etc).
• Arrival date/time.
• Departure date/time.
• Details of accommodation in Guernsey i.e. Hotel name.
• Special needs (if any).
• Medical details (if any).
• Dietary requirements (if any).
• Passport photograph for the form (if you could send this electronically please as my scanner is down).
• If we organise a swimming supporters "T" or Polo shirt would you like one if so please state size and preference.
